lifekeeper error: DEVICE FAILURE on SCSI device '/dev/add'
Since May 12,2009. Our system lifekeeper has the error log "lifekeeper error: DEVICE FAILURE on SCSI device '/dev/add'", but it ran normally. Until last week, it failover to the standby server. The disk still running, the error still come out.
Our Hardware guy checked the hard disk log and fabric, said nothing wrong.

You don't say what version/distro of Linux you're using, or where you're getting the error, but it seems pretty clear. "DEVICE FAILURE" indicates some sort of hardware problem. The fact that it failed is a good indication too.
If you post more details about your environment, software, etc., maybe we can help. But now, it seems like it's a hardware problem based on the log/symptoms, and that your hardware guy missed something.

Linux 2.6.18-53.el5 #1 SMP Wed Oct 10 16:34:19 EDT 2007 x86_64 GNU/Linux
Hardware: HP Proliant DL380 Server
DISK Storage: MSA 1000 SAN
The error message is from the lifekeeper log command "lk_log log"
Our hardware guy checked the HP Homepage,not seeing anything wrong from the server.

Ok, let's walk the diagnostic chain:
- DEVICE FAILURE messages appearing in logs since May (it is now September)
- Total device failure, causing the use of the backup unit.
Both of those point to a hardware problem. Sorry, but SOME piece of hardware has broken.
