HBA attached to my HP proliant running RHEL5.5 32 bit not working

My server has one emulex HBA card attached to it. I am trying to attach a new card to this server. at any point of time it is taking only one card and the dmesg shows the second card failed to setup.

LSPCI output

# lspci |grep Fibre
06:01.0 Fibre Channel: Emulex Corporation LP952 Fibre Channel Host Adapter (rev 01)
0a:01.0 Fibre Channel: Emulex Corporation LP952 Fibre Channel Host Adapter (rev 01)

message from log file

# grep hba /var/log/messages
May 21 16:14:54 ithrac2 kernel: lpfc 0000:06:02.0: 1:1477 Failed to set up hba

lpfc module details

# lsmod |grep lpfc
lpfc 406497 0
scsi_transport_fc 44873 1 lpfc
scsi_mod 141973 7 scsi_dh,usb_storage,lpfc,scsi_transport_fc,libata,cciss,sd_mod

# modinfo lpfc |grep version
version: 0:8.2.0.63.3p
srcversion: CABE989E4BB61146A4D4D5D

entries in modprobe.conf

# cat /etc/modprobe.conf
alias eth0 tg3
alias eth1 tg3
alias scsi_hostadapter cciss
alias scsi_hostadapter1 ata_piix
alias scsi_hostadapter2 lpfc
alias scsi_hostadapter3 usb-storage
options hangcheck-timer hangcheck_tick=1 hangcheck_margin=10 hangcheck_reboot=1

Please help me in configuring the new HBA and let me know whether any more details required. Also help me if any changes have to be made in modprobe.conf or in BIOS.

Chances are there might be some important information in the 10-20 lines preceding the "Failed to set up hba" message. Could you post that part of the log?

This could also be related to PCI IRQ allocation, in which case the error might appear quite early during the ACPI initialization phase. The output from dmesg should show all relevant information.

Have you tried updating both the adapter firmware and the ProLiant BIOS to the latest available versions?
