ssh on internet

how to ssh to a linux system through the internet while u r having ip address and hostname of that system.

erm.. how do you ssh to a box who's IP address and hostname you know???

ssh 12.34.56.78
ssh hostname.example.com

surely i'm missing something here? maybe it's because you didn't actually ask a question, just half demanded information without saying please or thankyou...
