Which specific Mail Transfer Agent you are using.
If you are using sendmail/qmail/postfix then probably Spamassasin ( open source ) is very effective.
Alternately at proprietery level, trendmicro IMSS is has the best effect which i have seen till now.
Every Mail transfer agent has its own advantages / disadvantages.
Well speaking off on scalability and security postfix and qmail are good alternatives however qmail is queiet complex to configure. For having a test setup before going live, you can check out http://www.qmailrocks.org which gives a complete explaination of setting up qmail server along with spamassasin running as antispam.
You can have spamassasin or any proprietery antivirus integrated to your existing sendmail setup also.
Qmail rocks for me If you need any help lemme know. One of the spam issues are related to "Dictionary Harvest Attack" (So allow Relaying only after some authentication like SMTP after POP AUTH or just SMTP AUTH). Also make note of "rblsmtpd" and http://www.spamhaus.com (google is your friend)
