Did the machine do this suddenly? If so: from when on? Was anything (re)configured, changed, updated, installed? Does it exhibit this behaviour intermittently or always? For a single or all users? Can root change passes for users? What are the contents of the password and sys-auth stacks in /etc/pam.d/? Does syslog (secure, messages) show anything?
