How to solve the problem of HTML_IFRAME.SMDM

Hi,
From a web developer' s point of view, how do u solve the problem of HTML_IFRAME.SMDM in a website? What should a web developer do so that a web site doesn't show "reported attack site" because of HTML_IFRAME.SMDM. What changes should be made in the code of the web site?

Suggestions? Solutions?

I believe what you are talking about is where a hacker has to insert that iframe code through another vulnerability. So to prevent it, fix the other vulnerability.
