It means that you can insert ethernet cord while logged in and it will detect network without reboot.

hotplug or hot-pluggable term is used for device that can be addded or removed while machine is in running state. So 'allow-hotplug eth0' means allow addition or removal of network card i.e. eth0 while machine is up and running.

This about the network cable, not the network card.
