unable to format sd card


I am in GParted, and I have a 16gb sandisk sd card. It shows up as only 30.61 MiB. Why isn't it showing up as 16 gb, and why can't I format it? when I right-click on it, and click new, it says that no partition table is found. i create a new partition table, and it looks like its working. I then click on new again, and it gives me the same error message about no partition table.

What's the output of fdisk -l?
How are you accessing the SD card (internal reader, usb adapter, etc.) and are you sure that it's working (SD card tested in another computer, reader tested with another SD card)?

There is not output.
I am accessing it via an sd card reader (internal). it gets REALLY hot - i actually burned myself. I'm thinking it's hardware.
