SD Card Read Problem (Not Mount Problem)

i ran

$ dmesg | tail

after installing my SD card (PQI 60) and got the following result:

sda: Mode Sense: 03 00 00 00
sda: assuming drive cache: write through
sda:<6>sd 0:0:0:0: Device not ready.
end_request: I/O error, dev sda, sector 0
printk: 2 messages suppressed.
Buffer I/O error on device sda, logical block 0
sd 0:0:0:0: Device not ready.
end_request: I/O error, dev sda, sector 0
Buffer I/O error on device sda, logical block 0
unable to read partition table

i had used this SD card on mepis before and it worked fine. does anyone else know what is going on? it seems the file system got mangled somehow - but i didn't do anything unusual with it.

tia...
