problem with a raid 1+0

Hello everyone
I have on a client the following issue.
In a production equipment with RHEL 4 we have configured two disk on raid 1+0 and if a disk goes down, we replace the disk and then might starts the copy from the good disk to the new disk, but sometimes happens that the machine does the copy from the new disk to the good disk! :S Then we lost every data......
What could be happen?

Is the raid set in the bios or are you using Linux raid?

Thanks for your answer! Well, the raid was done by the card that controls the disks.
EDIT: we are going to do a test with two disks, then what I need to do to keep the system on safe before I do the test? Could I do an image from the disk instead of a full backup? Both? How could I do the image?
