Luks passphrase

I encrypted my hard disk with LUKS , the LVM which was my /home ,
During boot time ,it doesn't receive the password and asking again and again 3 times ..
I use RHEL5 Client.
Before asking for passphrase ,its giving an error as

starting disc encryption , command failed :

What should I do to recover ???
