Question Kernel panic: VFS : Unable to mount root fs on 16:05

Hi there, I am new in linux and I experinced the following :
1. "Kmod : failed to exec /sbin/modprob -s -k SCSI -hostadapter, error =2" and
2. "Kernel panic: VFS : Unable to mount root fs on 16:05". Can it be a hardware problem or just something else. Because it was working fine before, and the hardware was removed and put back again.
Your help will be much appreciated.
