Question Gnome - making & using custom icons?

I was searching the web for a tutorial but didn't have much luck. I'd like to make some custom icons and use them in Gnome. My two main questions are:
1. What size and format should they be? 48x48?
2. Where would they be installed so that they can be chosen in the default Gnome "choose an icon" dialog?
