Should i change ISP?

My download is 10.88mb and upload is 5.40mb

This problem is that my packets arrive in strange time intervals. My jitter is 181ms . I can play streaming music and make voip calls if I use a large data buffer. Gaming is out of the question.

Very often the problem with packetloss and slow pings are in the internal network.
