sendmail adduser sbin/nologin using php

hi ....
we have a problem relating to php.....
i dont know much php and want to run the
system(adduser -s /sbin/nologin $user; passwd $new_pass) command to add users and passwds using add user command
we intend to make textboxes.....but dont have enugh knowledge abt them....
if anybody could tell how to make txtboxes (username and passwd )
and a submit button
and then to run the add user command....
it wuld be of great help...
thanx in advance
