I just googled that card # and didn't find much helpful (or encouraging), what's the output from the command line of:

lspci

and

cat /proc/pci

This is a Yamaha card, who tend to juggle chipsets constantly, so the lspci will actually tell us what chipset is on the card as opposed to what serial number Yamaha is calling it this week.

I have to warn you though, Yamaha is being one of the proprietary twerp companies lately, so a lot of driver support for their hardware is iffy at best due to the lack of info the Linux hackers have. For instance, my card is a Yamaha and goes berserk every once in a while.
