add header/footer to all pages via apache proxy

I'm running Apache 2.0.54 on Fedora 4 with mod_proxy enabled. I would like to have Apache add a header/footer to all pages that it proxies. I have tried mod_layout with no luck. Could someone point me to some other module or extension or whatever that would allow me to do this successfully?
