How to create a Patch like this?

How to create a patch like this?
readlist.com/lists/vger.kernel.org/linux-kernel/66/331739.html

A diffstat in the top followed by a
"Index: linux-2.6.21/file....
==================================================================="
for every file.
I've tried git format-patch and RCS. How to get that "Index:", line of "===" and the date + time of every file?

Thanks

since you have had not many replies so far....are you trying to read these messages or trying to make a patch out of a message?

if the latter, why not just copy and paste or grab the patch from the download link.

I use a vanilla patch from kernel.org but I am not sure what you are trying to do?
