QLA2XXX driver on Debian, Abort command issued
 
We are having problems using our QMH2462 4Gb Fibre Channel HBA on Debian Lenny 64bit with latest binary kernel image (2.6.26) and official, stable qla2xxx driver module (qla2xxx.ko: 8.02.01-k4, firmware: 4.04.00 and other 4.04.XX builds available at ftp.qlogic.com).

Basically the problem is that accessing hardware, e.g. using 'fdisk -l', hangs process for about 1-2 minutes while probing QMH2462 devices.
At the same time syslog is filled with "Abort command issued" entries.

Does anyone have any ideas what could be the problem?

It looks like problem was caused by failing hardware.
HP engeneers performed test and as a result some components have been replaced.
No problems afterwards.

storage controller?
qlogic card?
