Intel536EP Modem


I install my Intel536ep modem in debian with the following command

mknode c 536ep0 240 1
ls -s /dev/536ep0 /dev/modem
i have two question?

1- when i reboot my system i must do above command for working my modem,how can i fix it for all times

2-i set /etc/wvdial.conf and write my username and password like
username= test
password= 12345

when run wvdial in command it connect to my isp and display username and password without anyting and disconnect,and redial again...... why??
i cancle it with ctrl+c
