NVidia are wonderful people who even like to troll around their users forum. Try the nv forum
and scroll down to their linux section.

About not having video during boot-up, some cards do not activate TV-out during boot-up. This was the case of my old Geforce 2 MX400, it doesn't show anything on the TV during boot-up but once the Windows driver/NVTV on Linux is loaded the TV display comes on. It has a Chrontel TV encoder chip.
My current Geforce FX 5500 does show video during boot-up. It has a NVIDIA encoder chip. It even seems not to be supported by NVTV at all - I can't use it to control TV-Out, so I'm stuck to editing xorg.conf each time I want TV-out on.
