Format and mount problem - External USB harddisk


I recently decided to format my external usb harddisk, so I can write to it using linux. The drive was NTFS before.

I started qtparted and formated the drive with the ext3 filesystem. After a while qtparted gave the following error:

There was a problem with mkfs.ext3

I tried to mount the drive with:

mount /dev/sda1 /mnt/usb

This works, but when I try to write something to the drive I get a message saying the filesystem is readonly. After that the LED which is on the USB-case stays amber. After the write attemt I'm unable to unmount the drive, umount gives the message "device is busy".
When I turn the drive OFF and ON the LED stays amber.

I've connected the harddisk directly to an IDE cable, and formatted it using knoppix. When my debian installation is booting, I get the following error:

pivot_root: no such file or directory
/sbin/init: 426: cannot open dev/console: no such file
kernel panic: attempted to kill init!
