base address and port address


i am attemting to install drivers for my 56k, which is an ISA modem, i found the drivers and they said for the ISA version of the modem to use Forced Parameters, asking for IRQ(which i have) and Base address and Port Address.

which look something like 0x2f8

where would i find this information on my Modem?
