Windows spawning off screen

I'm not sure how this started to happen but on this particular system I have running (dual monitors, latest fluxbox, x.org), whenever a window is spawned on the second monitor, it always appears slightly off screen, with the title bar not visible. It has to be dragged down in order to be viewable.

How do I stop this from happening, where can I tell X to spawn windows in the middle of the screen or approx. middle?

I've only seen this issue with Java-apps in Fluxbox, and that's
because it starts to display the client are and then seems to
create the frame ... does this happened for EVERYTHING you start
or just particular apps?

Yup, everything including terminals.
