You can use cdrdao (
http://cdrdao.sf.net ) to create a .bin/.toc from a cd by the 'cdrdao read-cd --read-raw' command line. As far as can tell, the .bin created is the same as in .bin/.cue pairs. You may have to make a .cue file by hand from the .toc file - I don't know how else it might be done.
