Developing auto registration Form for Mail server

Hi Guyz,

My setup is
Postfix - MTA

Squirrelmail

Apache

I have set up a mail server, having user information, login and password is stored in Mysql.

I wanted to know how to create Auto registration form by which user can visit and create their own email Ids.

Is any one aware of this ?? then please let me know how to achieve this
