So it saw the disk, but never interrogated it. My card reader is similar. Connections? Cabling? Contacts? Overcurrent (Switching the power off)? There's a few ideas. I'd suspect a connection problem, or overcurrent. Linux doesn't yell about overcurrent - it just switches the port off. The ehci_hcd module was plagued by overcurrent issues at one stage. Card readers have an sdcard, a few leds, and an internal circuit - it could easily be tripping overcurrent.

The problem was that I was using a Mini SD card that takes an insert of a Micro SD into it.
The Micro SD was missing.
Replaced the SD card and it works just fine.
