tomcat based website not working

I have a website hosted on Linux Based VPS using tomcat.
Website after every 3 to 4 days it goes down and in browser it shows " Connect Timed Out"

Here the log of file /var/log/httpd/error_log :

Following line comes in log whenever site not working :

Any suggestion how to solve this .
Please help me to solve this.

This is just a warning, it's not harmful. To stop seeing that, you can add in httpd.conf:
JkShmFile     /var/log/httpd/mod_jk.shm
Regarding tomcat dying unexpectedly, could be some poorly written application that eats all available resources. Perhaps you can take a look at catalina.out to find out more
