Looking for a device recommendation for portable medical triage

I need a portable, battery operated, server that can create a Wi-Fi intranet for use by triage personnel in very rural or storm damaged areas.

The ideal would be a battery operated (possibly ARM based) board with WiFi, USB, and the ability to take a 2.5" disk. It needs to run MySQL, a web server with PHP (Apache?), and a few other bits and bobs (DHCP and DNS servers).

It should fit in a fanny pack worn by one of the medics, and then the other medics and medical staff in range will use e-ink web browsers to talk to the server, entering patient information and prioritizing cases.

If anyone knows of an off the shelf SBC or complete package that would work for this I would be obliged. Any suggestions on the best distro to use would also be gratefully accepted.

Thanks...
