web based IP / Server management

Not at all linux related, but figured I'd post here anyway.
I am wondering if anyone here has come across what I am looking for.
What I have at work is a lab environment that several people use. we are constantly installing, removing, rebuilding, etc various servers/applications/routers/switches etc. Currently we are managing our IP allocation for statically assigned devices in a spreadsheet, and a few documents for server/application access information, but this isn't working well. What I am looking for is a small open source (preferably PHP/MySQL based) app we can deploy on our LAMP server to better manage this all in one place accessible via browser.
ideally what I am looking for (I think) when i load the page I get a list of Subnets (we have a /16 network for our lab broken down into several /24 networks, The majority of our servers are in one /24 network, with the remaining networks using DHCP scopes for client devices). From here if I want to be able to click on a subnet and drill down and get a list of IP Addresses in that subnet, identifying what IP's are available, and what IP addresses are assigned to a server/application. (the ability to filter and show only assigned or unassigned IP's would be nice, but not required). from here I want to be able to click on an IP or application and drill down into a page that provides specifics on that application (version number, access information, etc. as every application requires some different information, i imagine this just being an open text field)
Well this will be hosted internally in the lab, I would still like it to be secure, requiring a user to login to view or edit information. The ability to link this with an Active Directory server for user login would be great, but not required.
an audit log would be a nice to have as well but not required (so we can see who created the server/app information, and who last edited it etc.)
Initially today I went down the path of using a WIKI for this, but best I can tell, this is going to require us to edit multiple wiki pages everytime we add a server, as such we will run into some of the same issues we have now managing it through spreadsheets and word docs (one doc gets updated, others don't).
