Had a USB stick that died for no apparent reason. The only thing I could think of was that I may not have "safely removed" the hardware from a Windows machine before pulling it. And based on the OP's original comment, the second USB device wasn't detected after using it on a Windows machine. I'm not saying that it's Windows' or Ubuntu's fault, but removing a USB while it's still being written to or before it's properly closed CAN cause it to be unreadable.
