You'll find the package for this theme here. You'll find the package I made for this here. It's big (70Mo) as it includes different wallpapers in several resolutions.
To create a new ksplash theme, I use the theme Default as template :
1. As root, copy the theme Default :

$ cd /usr/share/apps/ksplash/Themes
$ cp -R Default <My_Theme_Name>
2. In the new theme folder (ie <My_Theme_Name>) :
a. Edit the file Theme.rc, and :
1. In line [Ksplash Theme: Default], replace Default with <My_Theme_Name>.
2. In line Name=Default, replace Default with <My_Theme_Name>
3. Update the entries: Description, Version, Author, and Homepage, as you want.
b. There must be at least one background image named background.png (never tried, but should work with jpg) in a folder whose name is the image's resolution (i.e 1920x1080).
The Default theme has 5 resolutions (ie 1024x768, 1280x1024, 1600x1200, 1920x1080, 1920x1200). If you want to support all these resolutions, you'll need 5 versions of your background. Otherwise, you can delete the folders you don't need.
3. Test the new theme with the command ksplashx. Here you have the opportunity to take a screenshot of the theme (for the preview) using ksnapshot with a delay of 10-15 seconds :
