Questionnaire for a software based workshop

I have to make a questionnaire for a workshop, in which I will explain the working of a software, then which type of questions can I include in the questionnaire.
Secondly If I have a software manual, then how can I convert it into the format of a workshop

Once you can answer those questions maybe you can get a better feel for the type of questions. There is definitely an art to writing them. What form is the manual i.e. PDF etc. What software are you using for the workshop i.e. Microsoft PowerPoint etc. Are you wanting to show pages or excerpts of the manual? Do you have to worry about copyright laws etc?
