You should make sure that you have the following line in squid.conf

logformat common %>a %[ui %[un [%tl] "%rm %ru HTTP/%rv" %>Hs %<st %Ss:%Sh
Anyway, since you're running squid-3.1, you can also use the "emulate_httpd_log" option, like:

emulate_httpd_log on 
access_log /var/log/squid/access.log
