Try finding out in which package that file came from (with rpm commands?) and try to reinstal it.

I thought it was one of the pam modules. I have 5 other servers exactly the same so I copied the /lib64/security pam modules from the other server also the pam modules in /lib/security rebooted it still has the problem

Try setting access rights for that file, maybe user set for pam is not root? Maybe try giving write access to ALL for that file(s)?
And check other settings between servers that are related.
