MySQL Error 69

Strugling finding any info on the following error in MySQL:

Error executing query: Got error 69 from table handler

I am getting this on an sql which was preforming fine for over a year....

Any ideas on how to figure out what the issue is?

I am able to browse each table fine via phpMyAdamin running over the database.

Thanks
