Which driver do I choose?
 
Specs:
Vid Card: Geforce FX 5200
Linux: Mandrake 9.2

Do I download IA32 or IA64?

Unless you have a 64 bit cpu like an AMD Opteron or an Intel Itanium(unlikely), get the IA32 version.

Also, to avoid problems in the future, it might be a good idea to download the sources for the kernel you have.
