Preferred way to make a case-insensitive directory tree

What's the preferred way to create a case-insensitive directory tree?

Normally I would use ciopfs and I think it's great.

I did however, recently read the following:

The Linux User's Guide to Installing Mods on the [Baldur's Gate] Enhanced Editions

And it seems that an alternative method is to create a disk image file, create a case-insensitive filesystem on it (ntfs, vfat, or, I suppose, exfat), and mount that.

What do you choose?

The use case is, of course, running games whose mods expect a case-insensitive filesystem.

If you're feeling adventurous, and can cope with using ext3 and the 2.6.32 kernel:
