PAM authentication failed

Hi, I seem to have some troubles with PAM. Because :

Code:
[root@thabox /etc/pam.d]# usermod -G audio sohail
usermod: PAM authentication failed
How come I get this error? Note that i'm running usermod as root. I googled for this problem but I didn't get any were. Some help would be nice.
Oh and this is how my /etc/pam.d/useradd looks like :

Code:
[sohail@thabox ~]$ cat /etc/pam.d/useradd
# Begin /etc/pam.d/useradd

auth sufficient pam_rootok.so
auth required pam_unix.so
account required pam_unix.so
session required pam_unix.so
password required pam_permit.so

# End /etc/pam.d/useradd
