Spam filtering is not perfect. It uses a database of previously scanned email, and a combination of other methods such as RBLs, etc... Then once a mail is received by a package like Spamassassin it rates the email based on various criteria as to the likelihood that it is spam based on your settings that score will allow the email to pass, be marked as spam, or rejected as spam. You can increase the efficacy of a spam filter by providing it with known Spam, and known ham to help it better rate incoming emails.
