I wrote a document using LibreOffice Writer. It has heading styles and a generated table of contents.

I saved the document to ODT and as PDF.

I loaded the PDF into Caliber as an e-Book.

I converted the PDF to EPUB format for Nook(tm) devices.

I stored the EPUB to my 'droid phone and loaded it into
an EPUB reader.

VIOLA! I can see the table of contents as page #1.
Sadly, I cannot see any other pages in the EPUB.

I can read the original PDF just fine.

Q1. What must I do along the way so that I have a useful EPUB?
(I don't need fancy just complete and readable.)

Q2. Are their things that I need to do to the PDF in Calibre?

Q3. Are there things I need to do in LibreOffice Writer or
to the generated PDF before I convert it to EPUB with Calibre?

I'm using Calibre v 0.8.3 and LibreOffice 3.3.2 in Ubuntu Linux. I am able to add an .odt file into Calibre and convert to epub. I also tried saving the LibreOffice document as an Aportis pdb and inputting that into Calibre and then converting, but that did not look as good as the .odt to .epub conversion.

I'm trying this with rather simple documents that have no headers; so I'm not sure how the TOC would come out. I have found pdf to epub conversion to be the most problematic; so I would suggest you take pdf out of the equation where possible. In this case, odt should work, but it may not work in the earlier versions of Calibre. You might want to try save as .doc, because I am pretty sure earlier versions of Calibre converted .doc. Or best of all, you can get the latest version of Calibre for Linux or Windows at:

My ODT has Header 1 and Header 2 as well as numbered and un-numbered lists. I also have a generated table of contents.
I'll let you know how this turns out.

Do you or others know of a Barnes & Noble(R) Nook(tm) or Nook Color(tm) compatible e-book reader for linux (Ubuntu Lucid or newer)?

The Nook uses epub format, but the books from B&N have DRM; so there is no ereader in linux that I know of that reads them. It is possible to remove DRM, but that is not a part of any of the ereaders I know of. I use FBReader for non-DRM epub in Linux.
