How do I install qmail-queue patch post install of qmail?

I have a qmail smtp server up and running with tmda. I wish to integrate clam antivirus with it using qmail scanner. It requires the qmail-queue patch to the Makefile. If I understand it correctly, i would have to patch and then recompile? How do you apply the patch and wont recompiling break my curent configuration or overwrite my configuration files? Which part of the installation process do I have to redo? Thanks
