squid "access.log" access denied no user name

Respected Sir,
I had configured squid. It is taking the ip and the name of the user in the access.log who are browsing the internet.
but if any site is banned. and the user tried to access that site the access.log is not giving the name of the user.

I will be very thankful if any body can help me out.
Regards
Jatender
