I should say first of all that I am NOT familiar with Qmail or phpQLAdmin, but I am familiar with php, MySQL, and roundcube mail.
When you say you are "not able to run phpQLAdmin" what exactly do you mean? Do you see an error displayed in your web browser?
Have you checked your log files? Especially /var/log/messages, /var/log/httpd, and /var/lib/mysql/mysql-bin.err or the equivalents on your server. Also check /etc/php.ini and see if you have error_log set; if so, check that log file. If not, your php errors will be in your main syslog file (/var/log/messages on CentOS, which is what I'm familiar with).

While i am entering user name and password of ldap, then i am getting below error
Not Found

2) nice to hear that you are familiar with roundcubemail, i am also try to configure roundcubemail too but no luck, i am also attaching snapshot for that :
