make sure your LDAP server is up and running so that the phpldapadmin can connect to your ldap server and the login credential it is asking is of your root account as it has been said earlier.

Is your LDAP server configured, up and running properly as the phpldapadmin will try to connect to LDAP server through your machine.
change the default values
Code:
cn=admin,dc=examble,dc=com
to the one you have used while configuring LDAP
