PHP Form Generator


I would appreciate recommendations (and perhaps experience) for a relatively easy-to-use PHP form generation package -- it doesn't have to scratch my nose for me, but a basic form-from-table(s) would be nice.

I've been looking at phpFormGenerator and phpFormMail generator and I'm not sure that either will do; if anybody remembers (or is still using!) Informix' ISQL form (and report) generator (you simply generate a form from a table), that sort of thing would be the berries. Basically, I want to click or type one or more table names, get a basic form that I can edit and get on with life -- does such a thing exist in some form or other?
