Unhappy Load balancer configuration architecture


I have 2 application serves with 2nodes in each server having 2different port number.

Server 1 = N1:16001 & N2:16002
Server 2 = N3:16001 & N4:16002
Two Databases.
All 4 ports points to primary database.
I wanted to configure load balancer with fail-over with one URL.
Suggest me, I don't want technical procedure. I want only an idea.

We need to configure load balance between two nodes in server1 similarly in server2. And a fail-over between server1 & server2.
