Log user commands and output

Hello

Are there any tools/daemons/applications available to log user input (commands etc) and system output (stdout)? This is not to invade on anyone's privacy, but a security requirement. Any leads on this front will be much appreciated.

Regards
-r

And please elaborate what standard or policy this security requirement is based on?
