VG Shows boot partition unknown


I loaded clearos on server and after sometime for some reason I can boot into CLearos, I tried vgscan and it only picks up one partition, the other one is missing, seemingly my boot partition is missing its UID name and vg doesnt pick it up. If I do a vgdisplay it tells me that couldn't find device with uuid


Can someone please assist me in solving this issue
