PAM authentication failing

Hi I am using pam to authenticate my ssh session. Config is like this
if the user exists as linux authenticate via it otherwise use the custom pam module to auth the user.

config of /etc/pam.d/sshd is like this
auth sufficient /lib/security/pam_stack.so service=system-auth debug
auth required /lib/security/pam_meru.so debug
auth required /lib/security/pam_nologin.so debug
account required /lib/security/pam_stack.so service=system-auth debug
password required /lib/security/pam_stack.so service=system-auth debug
#session required /lib/security/pam_stack.so service=system-auth debug
session required /lib/security/pam_limits.so debug
session optional /lib/security/pam_console.so debug

My custom module returns a success to pam but pam says Authentication failed. Please let me know if I am missing something here

Hard to say, since we lack ANY details; things like version/distro of Linux, version of SSH, whether this works WITHOUT your custom module, or what exactly your custom module does or any details about it. We can't help without details.
